SPONSOR PLEDGE INSTRUCTIONS
This form is for check sponsors. For cash donations, please contact the HUYS Treasurer in advance.

1. Click here to download and print the form.
2. Complete the Team information section.
3. Ask the sponsor to complete the Sponsor Name/Address section and make the check payable to Henderson United Youth Soccer.
4. Mail the completed form and check to the address listed below.
5. HUYS will mail a check, in the same amount received, to the head coach of the team being sponsored, along with a receipt listing HUYS’s Federal Tax ID.
6. The head coach will need to provide the receipt to the sponsor.
