2. Complete the Registration Form and the Preference Sheet. Please make sure to SIGN the Liability Waiver and Medical Release Sections.

3. Bring form with you to one of the ON-SITE REGISTRATIONS or follow the instructions below for MAIL-IN REGISTRATION.
