1. Click on the HUYS Coach Registration form and S.A.Y. Volunteer Application below (PDF Format) and print out the registration form. BOTH forms must be filled out.

2. Complete the Registration Form and the S.A.Y. Volunteer Application. Please make sure to SIGN both forms.

3. Include (2) 1" x 1" Photographs of the Coach Applicant.
